Together, you’ll discuss the Personalized Action Plan, ensuring that the recommendations fit your lifestyle and needs. The specialist will help you prioritize the steps that will make the most immediate impact on your well-being. - Exploring Your Unique Strengths:
During the coaching session, you’ll work on uncovering your unique strengths—personal habits, skills, or routines you can leverage to kick-start and sustain your action plan. - Setting Goals and Tracking Progress:

How much does it cost?
Access to the Foundations Program is $165 and includes a comprehensive Assessment, Report, and Action Plan. You’ll also receive two personalized sessions with your Care Team to ensure you’re fully supported on your journey.
- Session 1: Meet with a Health Education Specialist to review your report, discuss any questions, and clarify your chosen next steps.
- Session 2: Work with your Health Coach to identify your strengths and finalize any adjustments to your Action Plan, ensuring you have all the tools and guidance you need to confidently take control of your health.
Add-ons and Upgrades
- Foundations members enjoy a 10% discount on ad-hoc sessions with Program Specialists and exclusive Health Coaching packages.
- If members decide to upgrade to Empower or Resilience within 3 months, the amount they've already invested will be credited towards the upgrade cost. For example, if a member has spent $350 on services and chooses to move up to the Resilience program, their upgrade cost will be reduced by $350.
